Licensed Social Worker Ambulatory Kailua Kona - Full-Time Day Shift

Conduct psychosocial assessments, diagnostic impressions, and case coordination for patients, families, and healthcare providers. Develop and implement social work service plans while managing casework interventions, resource referrals, and advocacy services. Serve as a liaison between Kaiser Permanente and community agencies to integrate care and enhance relationships through public relations activities. Evaluate practice effectiveness, perform clinical record audits, and participate in quality improvement processes and departmental committees. Maintain educational records, conduct research on social systems, and assist with annual work-plan development in full-time day shift rotation.
