Job Summary
Oversee resident admissions, identify needs, and make referrals to community resources while ensuring mental and social health. Develop relationships with residents and responsible parties for counseling, social interaction, and reality orientation. Assist with financial needs, complete acute care plans, and attend care plan meetings. Manage resident and family concerns, complete monthly QA reports, and refer residents for vision, dental, and hearing services. Conduct suicidal/homicidal assessments, assist in investigations, and complete MDS documentation. Discuss advance care planning and perform psychosocial assessments. Make morning rounds, attend committee meetings, and provide daily department updates. Work rotating weekend Manager on duty. Bachelor's or Master's in Social Work with Texas licensure required; skilled nursing facility experience preferred.
