Job Summary
Address emotional, spiritual, and psychosocial needs of terminally ill patients and families in collaboration with the transdisciplinary team. Assist with group counseling, discharge planning, community resource referrals, and Medicaid applications while maintaining accurate records and reports. Participate in care conferences, bereavement programs, chart reviews, and quality improvement meetings. Supervise Social Work Interns and coordinate with volunteer staff. Maintain Transmission Based Precautions and exhibit flexibility for evening, weekend, and on-call rotations. Complete annual competency requirements and assist with orientation and in-service programs.
Required Qualifications
- Masters prepared Social Worker
- Licensed in the State of Ohio
- Experience (2 yrs.) in counseling or related work with individuals, families and groups
- Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR's insurer
- Ability to provide proof of eligibility to work in the United States
- High level of integrity, ethics and professionalism
- Ability to provide exemption documentation for Annual Flu Shot Policy
Desired Qualifications
- Experience preferred in hospice, medical facilities or nursing homes
- Knowledge of community resources
