Licensed Social Services Director (LBSW or LMSW)

Conduct pre-admission and admission processes for clients, assisting them in adjusting to nursing home placement. Interview residents and families to gather behavioral and lifestyle history, then document progress and update care plans at interdisciplinary conferences. Arrange transportation and appointments while acting as a liaison between departments, families, and outside agencies. Develop discharge plans and coordinate post-discharge follow-up care. Organize Family Counsel Meetings, maintain minutes, and advocate for resident rights. Assess emotional, financial, spiritual, educational, and social needs, ensuring alignment with facility or outside agency protocols. Maintain effective communication with residents, families, healthcare professionals, and staff. Requires Bachelor's or Master's degree in Social Work with a state license and one year of supervised healthcare experience.
