Licensed Social Services Director (LBSW or LMSW)

Assist in pre-admission and admission processes, interviewing residents and families to adjust to nursing home placement. Document progress, update care plans at interdisciplinary conferences, and arrange transportation and appointments while acting as a liaison between departments, families, and outside agencies. Develop discharge plans, organize family counseling meetings, and maintain minutes. Assess emotional, financial, spiritual, educational, and social needs; ensure residents exercise their rights and advocate for them. Maintain effective communication with residents, families, healthcare professionals, and facility staff. Requires a Bachelor's or Master's degree in Social Work with a state license and one year of supervised experience in a healthcare setting.
