Licensed Social Services Director (LBSW or LMSW)

Assist in pre-admission and admission processes, including interviewing residents and families to understand past adjustments, behaviors, and lifestyles. Document progress, update care plans at interdisciplinary conferences, and arrange transportation and appointments while acting as a liaison between departments, families, and outside agencies. Develop discharge plans, organize family counseling meetings, and maintain minutes. Assess emotional, financial, spiritual, educational, and social needs; advocate for residents' rights and ensure they exercise them. Maintain effective communication with residents, families, healthcare professionals, and facility staff. Requires a Bachelor's or Master's degree in Social Work with a state license and one year of supervised healthcare experience.
