Licensed Social Services Director (LBSW or LMSW)

Conduct pre-admission and admission processes, assisting clients in adjusting to nursing home placement. Interview residents and families to gather behavioral and lifestyle history, then document progress and update care plans at interdisciplinary conferences. Arrange transportation and appointments while acting as a liaison between departments, families, and outside agencies. Develop discharge plans and coordinate post-discharge follow-up care. Organize Family Counsel Meetings, maintain minutes, and assess emotional, financial, spiritual, educational, and social needs. Serve as a resident and family advocate to ensure rights are exercised and needs are met through facility or external agencies. Maintain effective communication with residents, families, healthcare professionals, and staff.
