Licensed Registered Nurse Case Manager

Track and manage the medical status of assigned Soldiers through military systems of record, scheduling profile exams and evaluating evaluations at medical readiness events. Action daily referrals from MEDCHART DOD systems by coordinating follow-up appointments with Army medical providers and requesting further documentation. Correspond with medical readiness officers and Soldiers to obtain information for case management files, reviewing lab results and educating personnel on lifestyle modifications. Manage the Soldier's case from condition identification through medical readiness or administrative separation, updating MODS and other military systems for immunizations, pregnancy designations, and medical warning tags. Run MEDPROS reports pre- and post-event to compare rosters and ensure service completion, while tracking permanent PULHES cases for IDES referrals. Prepare medical documentation for Line of Duty investigations and individual vouchers for Automated Voucher System requirements. Stay current on regulations impacting case management and medical records.
