Job Summary
Provide professional nursing care to patients per the plan of care and company policies. Assess, plan, and intervene using the nursing process; administer treatments and medications in the home setting; teach patients/caregivers self-care techniques; supervise LPN and Home Health Aide visits; maintain documentation and data collection; coordinate with case managers, physicians, families, and health-care team; participate in multi-disciplinary conferences and on-call/weekend coverage; uphold confidentiality and comply with Medicare/Medicaid/HIPAA regulations.
Required Qualifications
- Graduate from an accredited registered nursing program
- Licensed as a LPN in the state of practice
- Clinical practical nursing experience preferably in intensive care, pediatrics, rehabilitation or medical surgical nursing
- Home health experience preferred
- Ability to make home visits and provide nursing care in the home environment
- Ability to complete necessary documentation appropriately, legibly and timely
- Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
- Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
