Job Summary
Provide psychotherapy and psychosocial support to residents in skilled nursing facilities as a Licensed Professional Counselor (LPC). Conduct initial diagnostic assessments, develop individualized treatment plans, and deliver individual, group, and family counseling focusing on depression, anxiety, grief, adjustment, and behavioral health issues. Document sessions in accordance with Medicare and facility requirements, participate in care plan meetings with the interdisciplinary team, and collaborate with facility staff and psychiatric nurse practitioners to ensure coordinated care. Educate facility staff on recognizing psychosocial needs and appropriate referral processes. Offer flexible scheduling with consistent facility assignments and contribute as part of an integrated behavioral health team serving post-acute and long-term care residents.
Required Qualifications
- Master’s in counseling or related field (regionally accredited; CACREP preferred but not required)
- Enrolled or eligible for enrollment as a Medicare Part B provider
- Experience working with geriatric populations or in long-term care preferred
- Strong interpersonal, documentation, and communication skills
- Ability to work independently within a collaborative, team-based environment
