Job Summary
Provide individual and group psychotherapy to geriatric residents in subacute and skilled nursing facilities. Conduct assessment, diagnosis, and treatment planning for conditions such as depression, anxiety, adjustment disorders, and cognitive decline. Develop and implement evidence-based treatment plans tailored to each patient, and collaborate with facility staff, interdisciplinary teams, and family members to support care. Maintain accurate clinical documentation in an EHR system and monitor patient progress to adjust plans as needed. Role supports both part-time and full-time schedules with opportunities for sign-on bonuses; requires active LPC license, in-person service delivery, and strong English communication. Required Qualifications
- Active and unrestricted license as a Licensed Professional Counselor (LPC) in the state you are applying for
- Effective verbal and written English communication
- Proficiency with electronic health records (EHR)
- Strong organizational and documentation skills with attention to regulatory compliance
- Ability to provide in-person psychotherapy services at assigned facilities (in-clinic or subacute/long-term care settings)
- Ability to work with geriatric population in subacute or skilled nursing facilities
- Capacity to collaborate with facility staff, interdisciplinary teams, and family members
- Willingness to adhere to facility policies including vaccination requirements where applicable
