Job Summary
Conduct individual and group psychotherapy sessions for patients in subacute or skilled nursing facilities. Assess, diagnose, and treat emotional and behavioral health conditions such as depression, anxiety, adjustment disorders, and cognitive decline. Develop and implement evidence-based treatment plans tailored to each patient's needs while collaborating with facility staff, interdisciplinary teams, and family members. Maintain accurate clinical documentation in the clinician-designed EHR and monitor patient progress to adjust treatment plans as necessary. This role offers customizable full-time schedules with no administrative burden, including billing and credentialing handled by Senior Care Therapy.
Required Qualifications
- Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.
- Effective oral and written communication in English
- Basic proficiency with technology, including electronic health records (EHR).
- Strong organizational and documentation skills, with attention to regulatory compliance.
- Ability to provide in-person services at assigned facilities, up to 45 minutes.
- Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
- Full Vaccination and Booster Status may be required in some facilities.
Desired Qualifications
- Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
- History of treating Anxiety, Depression and Adjustment Disorders.
- Psych or Addictions experience is a plus.
- Excellent time management and organizational skills.
