Job Summary
Provide Therapeutic Behavioral Services and case management to residents in skilled nursing and assisted living facilities, collaborating with licensed clinicians to develop individualized treatment plans. Conduct care planning, linkage, and crisis-prevention interventions, and deliver services to individuals or groups. Train staff on triggers, de-escalation techniques, and care strategies while maintaining open communication with facility staff. Ensure compliance with HIPAA and regulatory standards, and contribute to continuous quality improvement within an interdisciplinary care team.
Required Qualifications
- Licensed Professional Counselor or Licensed Marriage and Family Therapist
- Master’s degree in counseling or marriage and family therapy
- Eligibility for a state license to practice in the state of practice
- Knowledge of behavioral health treatment planning and case management
- Ability to maintain patient confidentiality and adhere to HIPAA
- Strong written and verbal communication skills
- Experience in assessing residents in long-term care settings (skilled nursing or assisted living)
- Ability to collaborate with licensed clinicians and multidisciplinary teams
- Commitment to recovery-oriented care and ethical standards
- Knowledge of regulatory and organizational policies governing behavioral health care
