Job Summary
Provide Therapeutic Behavioral Services and case management to residents in skilled nursing and assisted living facilities. Work within a multidisciplinary team to develop individualized treatment plans, coordinate care, provide links to interventions, crisis prevention, and staff training on client triggers and de-escalation. Maintain confidentiality per HIPAA, communicate with facility staff, and adhere to regulatory standards. Opportunities may exist for future transition to providing counseling services. Must hold a Master’s degree in counseling or marriage and family therapy and be eligible for licensure.
Required Qualifications
- Licensure as a Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT)
- Master’s degree in counseling or marriage and family therapy
- Eligibility for licensure in the practicing state
