Job Summary
Provide Therapeutic Behavioral Services and case management to residents in skilled nursing and assisted living facilities as part of a multidisciplinary team. The role includes creating individualized care plans, delivering crisis prevention services, training facility staff on triggers and de-escalation, maintaining communication with facility staff, residents, and families, and upholding HIPAA and regulatory standards. Opportunities may exist for transitioning to counseling services. Licensure as a Licensed Professional Counselor or Licensed Marriage and Family Therapist and state licensure eligibility are required; Master’s degree in counseling or marriage and family therapy is specified. Strong skills in data gathering, analysis of patient needs, and evidence-based behavioral health interventions are required, along with robust written and verbal communication and ability to collaborate across health care disciplines.
