Job Summary
Provide Therapeutic Behavioral Services and case management to residents in skilled nursing and assisted living facilities, collaborating with licensed clinicians to develop individualized treatment plans, deliver crisis prevention and interventions, and train staff on client triggers and de-escalation. Conduct and coordinate care planning, maintain patient confidentiality and HIPAA compliance, and communicate with facility staff to address resident care needs within an interdisciplinary team setting. Opportunities may arise for transitioning to counseling services; maintain knowledge of regulatory requirements and adherence to organizational standards.
Required Qualifications
- Master’s degree in counseling or marriage and family therapy
- License eligibility to practice in the state of practice
- Licensed Professional Counselor or Licensed Marriage and Family Therapist
- Knowledge of behavioral health practice rules and ethics
- Ability to provide training in social and emotional well-being, stress management and life skills
