Job Summary
Licensed Professional Counselor (LPC) to join integrated clinical team in long-term care facilities. Provide individual, family, and group therapy; conduct psychosocial evaluations, screenings, treatment planning; support residents with depression, anxiety, illness adjustment, grief, trauma, and behavioral disturbances. Collaborate with nursing, social services, rehabilitation, and medical staff; participate in care plan discussions and facility meetings. Maintain HIPAA-compliant documentation using GNT; ensure ethical and regulatory compliance. On-site role with a Monday–Friday schedule, no evenings or weekends, and strong administrative/clinical support. Requires active LPC license and valid driver’s license. Strong communication, organizational, and interdisciplinary collaboration skills; comfortable working with geriatric populations and medically complex residents. Fully supports autonomy within a team-based, facility-centered care model.
Required Qualifications
- Active, unrestricted LPC license
- Valid driver’s license and reliable transportation
- Strong written, verbal, and interpersonal communication skills
- Ability to work independently while collaborating with an interdisciplinary team
- Comfortable navigating LTC environments
- Commitment to timely documentation and high-quality care
