LICENSED PRACTITICAL NURSE

Provide direct nursing care to terminally ill patients at the patient’s residence under the supervision of an RN; work with the Interdisciplinary Team to ensure continuity and comprehensive care; apply hospice philosophy and principles of death/dying; communicate with patients and families compassionately; perform professional nursing care using the nursing process; collaborate with patients and families to support physical well‐being and address coping issues; prepare timely clinical documentation per hospice standards; participate in on-call responsibilities including weekends and evenings; maintain confidentiality; monitor patient status and alert physicians as needed; oversee Serenity Hospice Home Health Aides in the facility; assess infection control for patient, family, and staff safety; report patient status to the RN Case Manager; contribute to high quality and safe care and ongoing performance improvement.
