Job Summary
Provide direct nursing care to patients in home settings under RN supervision, implementing the plan of care and administering prescribed medications safely. Deliver patient and family education regarding diagnosis, medications, and self-care while maintaining accurate clinical documentation. Observe patient conditions and promptly report changes to the RN or Clinical Supervisor, participating in interdisciplinary care coordination. Promote a safe, therapeutic environment for patients and caregivers. Requires an active California LVN license, minimum one year of professional nursing experience, current CPR/BLS certification, and a valid driver's license with reliable transportation. Pay is $35-$50 per visit for this part-time role.
Required Qualifications
- Active LVN license in the State of California
- Graduate of an accredited practical or vocational nursing program
- Minimum 1 year of professional nursing experience
- Current CPR/BLS certification
- Valid California driver's license
- Reliable automobile
- Auto insurance per state/agency requirements
- Strong clinical skills in patient care, observation, and documentation
- Excellent verbal and written communication skills
- Organized
- Self-driven
- Able to prioritize effectively
- The ability to sit occasionally for 1-3 hours
- The ability to frequently stand for 3-6 hours
- The ability to occasionally drive/ operate foot control for 1-3 hours
- The ability to walk intermittently on level and or unlevel surface for less than 1 hour
- The ability to intermittently bend or climb for less than 1 hour
- The ability to occasionally twist, kneel, crouch and crawl for 1-3 hours
- The ability to lift 50 pounds frequently for 3-6 hours
- The ability to lift greater than 50 pounds occasionally for 1-3 hours
- The ability to carry up to 50 pounds frequently for 3-6 hours
- The ability to carry greater than 50 pounds occasionally for 1-3 hours
- The ability to push up to 50 pounds frequently for 3-6 hours
