Licensed Practical/Vocational Nurse (LPN/LVN)

Job Summary
Initiate, perform, and complete assigned patient care duties for variable populations, including administering prescribed medications and recognizing urgent situations to seek RN or MD assistance. Prepare and administer treatments according to established policies, assist in diagnostic examinations, and monitor patient conditions before, during, and after procedures. Participate in daily MHP/MCMH team huddles to review appointments, identify resource needs, and scrub schedules for diagnostic procedures. Prepare electronic health records, coordinate referrals and hospitalizations, develop community resource lists, and establish relationships with consulting providers to ensure continuity of care. Work Monday through Friday from 6:45 am to 5:00 pm with a 30-minute lunch break; no on-call, weekends, or holidays.
Required Qualifications
- Certificate
- Graduate from an accredited community college, junior college, college, university or vocational nursing program approved by the appropriate State agency and accredited by the Commission on Nursing Education Accreditation (CNEA)
- Possess a current AHA or ARC BLS Healthcare Provider certification
- One year of practical or vocational nurse experience after graduation
- Minimum of 1 year full-time clinical experience within the last 2 years as a LPN/LVN
- Current, full, active and unrestricted license as a practical or vocational nurse
- Must possess ability to pass a Government background check/security clearance
