Job Summary
Full-Time Home Health LPN providing skilled nursing care in patients' homes across the Lebanon, OH area. Responsibilities include delivering professional nursing care to patients and families as prescribed by the physician and in compliance with Home Health Medicare Conditions of Participation and Ohio licensure laws. Requires graduation from an approved nursing program and a valid Ohio LPN license with CPR. Minimum 2 years of direct patient care experience in hospice or home care. Must be a licensed driver with a reliable automobile and valid insurance; driving record must meet insurability standards. Shift: 8:00 AM–5:00 PM, Monday–Friday, with some on-call rotation. Required Qualifications
- Graduate of approved nursing program
- Valid Ohio Licensed Practical Nurse (LPN) license
- CPR
- Experience: a minimum of 2 years’ direct patient care experience in hospice or home care
- Must be licensed driver with automobile and insurance in accordance with Ohio laws and have clean driving record
