Job Summary
Provide skilled professional nursing care to home health patients, families, and caregivers as prescribed by physicians while adhering to Medicare Conditions of Participation, Ohio licensure laws, and agency policies. Manage the primary caseload for skilled home health care patients throughout the greater Lebanon Ohio area, including Lebanon, Middletown, Springboro, Dayton, and part of Cincinnati. Maintain a valid Ohio LPN license, CPR certification, and a clean driving record with a licensed vehicle for community-based service delivery. Work full-time 8a-5p Monday through Friday with some on-call rotation.
Required Qualifications
- Graduate of approved nursing program
- Valid Ohio Licensed Practical Nurse (LPN) license
- CPR
- A minimum of 2 years' direct patient care experience in hospice or home care
- Licensed drivers with automobiles in good working conditions
- Automobile insurance in accordance with Ohio laws
- Clean driving record for insurability purposes
- Full-Time 8a-5p Monday - Friday
- Some on call rotation/responsibility
