Job Summary
Provide direct resident care and supervision based on individualized service/care plans, monitoring residents for changes in condition and signs of illness per federal, state, and community policies. Communicate and document resident concerns to the Health Services Director, assist in supervising the care team, and administer medications and treatments per doctor's orders. Assess, document, and report changes in resident condition, implement behavioral management interventions, and ensure resident privacy and dignity. Immediately intervene, report, and investigate suspected abuse or theft. Maintain safe medication storage, review vital signs, and ensure infection control procedures are followed. Reports to Health Services Director. Requires unencumbered LPN license with at least one year of assisted living experience, current driver's license, and CPR/First Aid certification. Night shift.
Required Qualifications
- A valid Licensed Practical Nurse (LPN) license
- An unencumbered LPN license
- Minimum of at least one year of successful experience as a LPN in assisted living or long-term care
- Experience administering medication and treatment plans per doctor's orders and community policies
- Knowledge of state regulations pertaining to resident care
- Understanding of the importance of being state survey ready, every day
- Experience working as a mandatory reporter
- Ability to immediately intervene, report, and investigate all suspected or alleged verbal, physical, sexual abuse, or theft from a resident
- A current driver's license
- Willingness to adhere to assisted living principles in provided services
- Ability to utilize and apply basic management principles
- Ability to recognize and communicate problems effectively
- Ability to relate to the public, Residents, families, staff, other professionals appropriately
- Ability to communicate well, verbally and in writing
- Ability to maintain accurate records
- Ability to adhere to and communicate policy and procedures
- Ability/willingness to improve skills of self and staff; to be active professionally
- Ability to lift to 50 pounds occasionally, 30 pounds regularly
- Maintains appropriate and required license
- CPR/First Aid certification
