Job Summary
Provide direct resident care and supervise residents' needs based on individualized service/care plans. Monitor residents for condition changes and illness signs, document resident incidents, and report suspected abuse or theft immediately. Administer medications and treatments per doctor's orders and community policies while ensuring implementation of care plans. Assist in supervising the care team and communicate care plan changes to resident assistants. Shifts include full-time, swing, or overnight rotations from 2:00 p.m. to 10:00 p.m. or 10:00 p.m. to 6:00 a.m. with pay of $33.00 to $36.00 per hour. Requires a valid unencumbered LPN license and at least one year of assisted living or long-term care experience.
Required Qualifications
- A valid Licensed Practical Nurse (LPN) license
- An unencumbered LPN license
- Minimum of at least one year of successful experience as a LPN in assisted living or long-term care
- Experience administering medication and treatment plans per doctor's orders and community policies
- Knowledge of state regulations pertaining to resident care
- Understanding the importance of being state survey ready, every day
- Experience working as a mandatory reporter
- Ability to immediately intervene, report, and investigate all suspected or alleged verbal, physical, sexual abuse, or theft from a resident
- A current driver's license
- Ability to lift to 50 pounds occasionally, 30 pounds regularly
- Maintains appropriate and required license
- CPR/First Aid certification
