Job Summary
Licensed Practical Nurse (LPN) needed for part-time, evening shift (6:00 p.m.–6:00 a.m.) at Trustwell Living. Responsibilities include direct resident care and supervision per individualized care plan, monitoring changes in condition, communicating with Health Services Director, and supervising care team. Requires active LPN license, at least one year of geriatric/assisted living nursing experience, medication administration per policies, knowledge of state regulations, mandatory reporting duties, and a valid driver’s license. Required Qualifications
- Licensed Practical Nurse (LPN) license (compact license encouraged or ability to obtain)
- unencumbered LPN license required
- minimum of at least one year of LPN experience in assisted living or long-term care
- experience administering medications and treatment plans per doctor's orders and community policies
- knowledge of state regulations pertaining to resident care and understanding of being state survey ready
- experience as mandatory reporter and ability to intervene/report/investigate abuse or theft
- current driver's license
- ability to lift to 50 pounds occasionally, 30 pounds regularly
- CPR/First Aid certification
