Job Summary
Licensed Practical Nurse (LPN) providing direct resident care and supervision per individualized care plans in an assisted living/long-term care setting. Monitor residents for changes in condition and signs/symptoms of illness in accordance with federal/state laws and community policies; communicate and document changes to the Health Services Director; supervise the care team; administer medications/treatments per policies; ensure resident privacy and dignity; maintain documentation of resident data and physician orders; support infection control and safety procedures; respond to emergencies; uphold compliance with regulatory requirements and resident rights; ability to lift up to 50 pounds occasionally and 30 pounds regularly; CPR/First Aid certification preferred.
Required Qualifications
- Licensed Practical Nurse (LPN) license with unencumbered status
- Compact license or ability to obtain one encouraged
- Minimum of at least one year of LPN experience in assisted living or long-term care
- Experience administering medications and treatments per doctor’s orders and community policies
- Knowledge of state regulations pertaining to resident care and focus on being state survey ready
- Experience as a mandatory reporter and ability to intervene, report, and investigate suspected or alleged abuse or theft from a resident
- Current driver’s license
