Licensed Practical Nurse Traveler

Job Summary
Provide nursing care to patients in their homes as a licensed practical nurse traveler, including administering medications and wound care; monitor patient conditions, report changes to the care team, and assist in developing personalized care plans; educate patients and families on health management, medications, and self-care techniques; collaborate with a multidisciplinary team to ensure high-quality home health care. Works under RN supervision and assists in implementing the plan of care across age groups (neonates to geriatrics); duties include assessment, planning, implementation, evaluation, effective communication per agency policies, care coordination, scheduling adjustments, and maintaining patient satisfaction. Requires graduation from an approved PN program, valid NC PN license, CPR certification, at least one year of nursing experience, and a valid NC driver’s license with access to a vehicle. Hours vary (Mon-Fri, weekends/holidays as needed); travel to patients’ homes in the Carolinas is expected. Required Qualifications
- Graduate of an accredited school of practical nursing
- Current license to practice practical nursing in the State of North Carolina
- CPR certification
- Minimum one year practical nursing experience
- Home health experience preferred
- Valid North Carolina driver’s license and an operational vehicle
- Ability to communicate in English and to read and write effectively
- Ability to transfer and maneuver patients and use care equipment
