Licensed Practical Nurse

Provide medical treatment and personal care to residents during 3rd shift (11pm - 7am). Administer medications and treatments, take vital signs, assist with assessments, and document in EHR. Maintain resident safety and comfort; assist with daily living activities; collect lab samples and follow up on results with residents, families, and physicians. Prepare and check food trays per prescribed diets. Maintain cleanliness and safety in resident rooms; respond to resident calls. Support and train staff under the Health Care Coordinator. Comply with HIPAA, company policies, and applicable regulations. Requires LPN license, minimum six months caregiving experience (elderly residents preferred), strong interpersonal skills, ability to work in a multidisciplinary team, and physical ability to lift up to 50 lbs.
