Job Summary
Licensed Practical Nurse in Home Health provides quality patient care in patients' homes within the agency's plan of care, following infection control procedures and coordinating with the health care team. Responsible for thorough documentation in the electronic medical record, participation in care coordination including timely reporting of patient needs and changes in condition, attending case conferences, and effective communication with patients, families, physicians, other clinicians, and management. Applicable for a home-health setting under the UnitedHealth Group umbrella with opportunities for development and growth within the role.
Required Qualifications
- Current and unrestricted LPN licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Ability to work flexible hours as required
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
