Job Summary
Licensed Practical Nurse (LPN) in Suffolk County providing in-home/home-care and community-based nursing care under RN supervision. Responsibilities include following physician plan of care, delivering treatments, documenting accurately and timely, educating patient and family on diagnosis and plan of care, recognizing symptoms and initiating remedial measures within LPN scope, observing patient condition and reporting changes, coordinating hospice and other home care services with the interdisciplinary team, participating in Hospice IDT meetings, delivering pain/symptom management, adhering to aseptic technique, and contributing to quality improvement efforts. Weekend and holiday commitments required. Benefits include 401(k), mileage reimbursement, health insurance, and PTO. Requires graduation from an accredited practical nurse program, current LPN license in the state, and 1-2 years of LPN experience (home care preferred). Alternating weekend days and willingness to commute for in-home patient care are noted.
Required Qualifications
