Job Summary
Licensed Practical Nurse/Licensed Vocational Nurse for community-based nursing in Sevita-affiliated REM Community Services. Part-time role (Thu 3p-11p and Sat 7a-11p) providing nursing services, training, and supports to individuals with brain injury, IDD, substance use disorders, and/or emotional/behavioral disabilities. Responsibilities include monitoring health conditions, administering medications, documenting per physician orders, overseeing staff administering medications, conducting assessments and implementing health care services in Individual Service Plans (ISP), training staff on universal precautions, infection control, and medication administration, coordinating with external medical providers, and providing training on topics such as self-medication, sex education/STD prevention, and smoking cessation. Requires effective communication with staff, individuals served, and families. Required Qualifications
- Associate’s Degree in Nursing or related field, or state certificate
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure for state of residence
- Current driver's license, car registration, and auto insurance
- Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
