Licensed Practical Nurse

Provide prescribed medical treatment and personal care services under RN or physician direction for individuals with developmental disabilities, brain injuries, and substance abuse issues. Complete health assessments, administer and record medications, and ensure follow-through on medical and dental appointments. Conduct quality assurance checks on health care records, review medication error forms, and partner with external agencies to coordinate services. Maintain confidentiality, practice universal precautions, and provide training to staff and individuals served on topics including infection control and self-medication. Participate in the on-call system for emergency coverage and assist with household activities, transportation, and meal preparation as needed. Travel between multiple locations while remaining accessible to staff during work hours.
