Job Summary
Licensed Practical Nurse/Licensed Vocational Nurse role providing nursing services, training, and supports to individuals with brain injury, intellectual/developmental disability, substance use disorder, or behavioral disabilities. Monitor health conditions, administer medications and document, supervise staff administering medications, provide care per physician’s orders and ISP plans, conduct assessments, and deliver staff training on topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation. Coordinate with medical providers and external agencies to ensure consistent treatment, and serve as liaison/advocate while maintaining high-quality care.
Required Qualifications
- Associate’s Degree in Nursing or state certificate
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure for state of residence
- Current driver's license, car registration, and auto insurance
- CPR/FA and HIV/AIDS certification, BBP, and ACT training as required by state licensing regulations
- Ability to effectively communicate with staff, individuals served, and families
