Job Summary
Licensed Practical Nurse with responsibilities including monitoring health conditions, administering medications, documenting care, supervising staff who administer medications, providing physician-directed treatments and personal care, conducting assessments, implementing health services in ISP plans, providing staff training on infection control and medication administration, and acting as liaison with medical professionals. Set within Sevita's REM Community Services, serving individuals with intellectual and developmental disabilities, brain injury, substance use disorders, or emotional/behavioral disabilities. Offers full-time, part-time, and as-needed schedules with a full compensation/benefits package and opportunities for career development.
Required Qualifications
- Associate’s Degree in Nursing or related field, or graduate of an accredited LPN program
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN License
- Current driver's license, car registration, and auto insurance
- Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
- Ability to effectively communicate with staff, individuals served, and families
