Job Summary
Provide nursing services and training within a community-based setting for individuals with brain injury, IDD, substance use disorders, and behavioral disabilities. Monitor health conditions and ensure quality services; administer medications and supervise staff who administer meds; implement prescribed medical treatments and ISP-driven care; conduct assessments and deliver healthcare services per ISP; train staff on universal precautions, infection control, and medication administration; coordinate with medical providers and external agencies to ensure consistent treatment; provide training on self-medication, sex education/STD prevention, and smoking cessation. Requires an Associate’s Degree in Nursing or state nursing certificate and an active LPN/LVN license with at least one year of relevant experience; valid state driver’s license, car registration, and auto insurance. Required Qualifications
- Associate’s Degree in Nursing or related field, or state certificate
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure for state of residence
- Current driver's license, car registration, and auto insurance
- Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
