Job Summary
As a Licensed Practical Nurse (LPN), you will provide nursing services, training, and support to individuals with medical or psychiatric needs. Responsibilities include monitoring health conditions, administering medications, documenting care, conducting assessments, and implementing health care services in accordance with Individual Service Plans. You will also provide training to staff and individuals served on various health topics, and serve as a liaison with other medical providers to coordinate services.
Required Qualifications
- Associate’s Degree in Nursing or related field, or state certificate
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure for state of residence
- Ability to effectively communicate with staff, individuals served, and families
Desired Qualifications
- Current driver's license, car registration, and auto insurance
- Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
