Coordinate shelter residents for ongoing community treatment by meeting with program directors, psychiatrists, and social services staff. Administer medications, document administration, and monitor medication records while taking vital signs and assisting with diabetes management. Conduct health assessments, maintain medical histories, and ensure physician orders are current and properly communicated. Monitor client safety, nutritional needs, and equipment functionality while completing daily documentation and incident reports. Refer residents to emergency care and coordinate discharge planning with hospital staff. Work evenings and weekends to ensure daily consumer safety and well-being.
