Job Summary
Monitor and follow patient progress, report health changes to supervising registered nurses, and deliver personalized care management by evaluating evolving patient needs. Coordinate quality-driven care within the interdisciplinary team, traveling to patients' homes or facilities to manage weekly caseloads using effective time management. Use technology to maintain accurate electronic medical records and provide empowering education to patients and families regarding end-of-life treatment. Practice within the LPN scope of practice while utilizing empathy to ensure continuity of care.
Required Qualifications
- License and current registration to practice as an LPN in New York State
- Valid driver's license
- High School Diploma or completion of LPN Program
- Bilingual skills
