Job Summary
Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures. Assist with activities of daily living, supervise nursing assistants, and monitor physical and emotional status to notify physicians of changes. Develop individualized care plans, complete clinical documentation, and coordinate admissions, transfers, and discharges. Participate in quality assurance activities, staff education, and regulatory surveys as needed. This role requires a current, active LPN/LVN license, CPR/BLS certification, and at least one year of clinical nursing experience. Long-term care experience and supervisory skills are preferred. The position offers up to $34.70/hour at Signature HealthCARE of Terre Haute, a 176-bed facility, with benefits including tuition forgiveness, shift differentials, and PTO.
Required Qualifications
- Current, active LPN/LVN license in the state of practice
- Minimum one (1) year of clinical nursing experience
- Knowledge of Federal and State long-term care regulations
- Effective verbal and written communication skills
- Ability to work independently, prioritize tasks, and exercise sound clinical judgment
- Familiarity with electronic health records (EHR) systems
- Proficiency with Microsoft Office applications
Desired Qualifications
- CPR/BLS certification (American Heart Association or American Red Cross)
- Long-term care experience
- Experience in a supervisory or charge nurse role
