Licensed Practical Nurse/Registered Nurse - Private Duty

Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases, often following hospital discharge, under RN supervision. Take patient vitals, perform assessments, document progress, and report changes to the RN supervisor. Assist in carrying out the patient's plan of care, teach self-care techniques, and educate families on medication and home safety. Collaborate with physicians, RNs, CNAs, and aides to coordinate care and plan for discharge. Work one-on-one with patients from 4:30 PM to 11:00 PM Monday through Friday, with weekends off. Join a nationwide network of nurses dedicated to patient advocacy and professional support.
Required Qualifications
- Graduate of an accredited practical nursing program
- Active RN OR LPN license in TN
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing
- Strong communication skills
- Compassion
- Must be available for 4:30PM-11:00PM Monday-Friday
- Must be available for off weekends
