Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases under RN supervision. Take patient vitals, perform assessments, document progress, and report changes to the RN supervisor. Assist in carrying out the patient's plan of care, teaching self-care techniques, and educating families on medication and home safety. Collaborate with physicians, RNs, CNAs, and aides to coordinate care and plan for discharge. Work Monday through Friday from 11 pm to 7 am with a 1:1 nurse-to-patient ratio. Required Qualifications
- Graduate of an accredited practical nursing program
- Active RN OR LPN license in TN
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing
- Strong communication skills
- Compassion
- Monday-Friday 11pm-7am
