Licensed Practical Nurse Psych Visits PRN

Licensed Practical Nurse Behavioral Health providing non-traditional visits in patients' homes (PRN, weekend field runs) to deliver skilled nursing care under RN delegation. Responsibilities include verifying the Plan of Care prior to visits, delivering nursing services per orders and standards, documenting in skilled nursing notes, communicating changes to the RN Case Manager and physician, ensuring patient safety, teaching self-care to patients and families, updating the plan of care as issues arise, and collaborating with the RN Case Manager during normal business hours. Requires LPN licensure, a nursing diploma/associate program, 1 year of clinical care experience (home health/hospice preferred), ability to travel up to 50%, and a valid driver’s license with reliable transportation. Compensation ranges from $30.00 to $32.00 per hour with potential additional incentives; base salary reflects annualized earnings with full benefits and equal opportunity employment statements included.
