Job Summary
Verify the Plan of Care prior to each visit and provide care according to delegation by the RN, physician's orders, and established standards. Incorporate patient care goals into nursing visits as evidenced by documentation on skilled nursing notes. Communicate significant findings, problems, and changes in condition to the RN Case Manager, physician, and clinical supervisor. Report unsafe conditions to appropriate personnel and implement the plan for patient safety using community resources. Assist patients in learning self-care techniques and suggest revisions to the plan of care based on identified issues. Treat patients and caregivers effectively while documenting the medication regimen and updating the medication profile. Implement actions to prevent avoidable hospitalizations and ER visits through positive communication with all agency personnel. This role requires 50% travel, a valid driver's license, and 1 year of clinical experience.
Required Qualifications
- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver's license and insurance and reliable transportation to perform job tasks
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
- EVerify participation (after job offer is accepted and Form I-9 completed)
