Licensed Practical Nurse Psych Visits FT

Licensed Practical Nurse Behavioral Health providing in-home visits in Ware/Palmer/Monson areas. Verifies Plan of Care and delivers nursing services under RN delegation, incorporating patient goals into visits, documenting in skilled nursing notes, and communicating changes to RN Case Manager/physician. Maintains patient safety plans, assists patients in self-care, suggests revisions to care plan, and collaborates with the RN Case Manager. Requires 1 year of clinical care experience, LPN license, travel up to 50%, valid driver's license, and the ability to lift up to 50 pounds. Compensation ranges from $30.00 to $32.00 per hour with full benefits including tuition reimbursement, medical/dental/vision, 401(k) with employer match, PTO/holidays, and equal opportunity employment. Reports to the Clinical Team Manager.
