Job Summary
Licensed Practical Nurse Behavioral Health for PRN weekend/evening field visits providing nursing care in patients' homes; verifies the Plan of Care, delivers visits under RN delegation, documents nursing notes, and communicates significant findings to the RN Case Manager and physicians. Implements patient safety plans, educates patients and families on self-care, and collaborates with the RN Case Manager to revise the plan of care as needed. Requires LPN license, graduation from an accredited nursing program (Certificate/Diploma/Associate), 1 year of clinical care experience (home health/hospice preferred), willingness to travel up to 50%, valid driver’s license, and ability to lift 50-100 lbs. Location: Springfield, MA area with PRN weekend/evening field visits.
Required Qualifications
- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting; hospice/home health preferred
- 50% travel required; valid driver’s license and reliable transportation
- Sit, stand, bend, lift and move intermittently; able to lift 50-100 lbs
