Job Summary
Licensed Practical Nurse Behavioral Health for home-visit visits (PRN) in the Hartford area, Evening and Weekend field runs. Provides nursing care in patients' homes, verifies Plan of Care, documents medication regimens, communicates with RN Case Manager and physicians, and supports patient safety. Requires LPN license, 1 year of clinical care experience (home health preferred, hospice preferred), and ability to travel up to ~50%. Required Qualifications
- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting; hospice preferred
- Experience in a home health environment is preferred
- Valid driver’s license and reliable transportation
