Job Summary
Assess patients' physical, psychological, and social dimensions in Health Care Units and housing units, contributing to individualized treatment planning. Implement treatment programs directed by Health Care Practitioners, administering medications including IV therapy (if certified) and performing diagnostic tests per protocols. Collaborate in controlled substance counts, respond to codes or health emergencies, and assist in medical or minor surgical procedures. Document nursing encounters using SOAP charting while attending mandatory staff meetings and training sessions. Work under RN supervision to provide direction in patient education and activities aligned with demonstrated competencies.
Required Qualifications
- Graduate from an accredited school of nursing
- Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment
- Must be able to obtain and maintain CPR certification
- Minimum of one (1) year clinic experience

- Prefer a minimum of one (1) year clinic experience
