Job Summary
Provide hands-on patient care as an LPN across diverse healthcare settings, including hospitals, skilled nursing facilities, rehabilitation centers, nursing homes, and assisted living communities. Deliver medication administration, wound care, vital signs monitoring, injections, blood draws, and catheter care. Admit, discharge, and transfer patients per facility policies; maintain accurate, compliant documentation. Collaborate with RNs and interdisciplinary teams to deliver personalized care; participate in team huddles and contribute ideas to improve outcomes. Ensure supplies are stocked and medications/equipment are available. Maintain effective communication with patients, families, and colleagues to foster a positive care environment. Adhere to CMS and local Department of Health guidance on vaccination/testing; comply with EMR usage and relevant certifications. Flexible per diem scheduling and a supportive FlexiCare staffing program are highlighted as part of the role.
Required Qualifications
- Graduate of an accredited Licensed Practical Nurse (LPN) program
- Active, unrestricted LPN license in the state of practice
- Minimum 6 months of recent LPN experience (1–2 years preferred)
- Current CPR/BLS certification
- Ability to push, pull, lift a minimum of 80 pounds
- Physical stamina to stand/walk for long hours
- Annual physical examination where required
- Negative TB test results
- Clean background check
- COVID-19 vaccination and boosters as required by CMS or state regulations
