Job Summary
Monitor chronic health conditions, admit and discharge residents, and conduct comprehensive assessments using MDS forms. Administer medications per orders, provide personal care and ADL assistance, and assist with repositioning and skin health monitoring. Collaborate on individualized care plans, oversee CNAs, and support end-of-life palliative care with hospice services. Maintain documentation via nurse's notes and electronic records while adhering to infection prevention and falls prevention protocols. Provide first aid and transcribe provider orders as needed. Employment requires a Florida AHCA Level 2 background screening.
