Job Summary

Provide direct patient care, implement nursing interventions, and document accurately according to the plan of care. Assess and educate patients and families on diagnosis and care plans while coordinating home health services with the registered nurse. Mentor direct care staff and participate in interdisciplinary team meetings to coordinate client needs and problem-solve. Use equipment efficiently and rotate on-call shifts. Maintain a professional manner, manage time effectively, and adhere to all facility policies, regulations, and HIPAA training requirements.

Required Qualifications

Degree in nursing field as a licensed practical nurse
Current LPN license in good standing in the state(s) in which practicing
Valid driver's license
Automobile insurance
Qualified to drive under the organization's motor vehicle check
